Our Financial Policy

1. All copays, deductibles, and account balances are due at the time of service.
3. We accept the following forms of payment:  DEBIT CARD, CREDIT CARD (Visa, MasterCard, Amex, and Discover), CHECK, and CASH. (Please note that the Dermatology and Skin Cancer Institute does not accept Amex or Discover.)
5. It is your responsibility to know your insurance benefits. We can never guarantee insurance coverage for any service provided. We will file claims to your insurance carrier and accept payment directly from them. It is the patient’s responsibility to keep us informed with up to date insurance coverage and contact information. Patients are fully responsible for all costs denied by their insurance. 

If your plan requires a referral or prior authorization to see the doctor, it is your responsibility to obtain this before your visit.  If your referral is not received prior to your scheduled appointment, you will not be able to be seen and will have to reschedule.

Received a bill for a $25 No Show fee? We ask that patients give us 24 hours notice (48 hours for surgical appointments) to cancel appointments. Failure to give sufficient notice will result in a $25 No-Show Fee, which must be paid before another appointment can be scheduled.

We accept most commercial insurance plans, including: Medicare, Aetna, Blue Cross/Blue Shield, Cigna, Keystone Health Plan East, Keystone 65, Personal Choice, United Healthcare, Tricare, UMR, Meritain, and more.

Plans we NOT accept include:

Humana, Keystone First, Keystone Mercy, Keystone Kids, Keystone Connect, Health Partners, Medicaid, Cigna Health Spring, Aetna Better Health, UHC Community, Access, Coventry, Bravo, Americhoice.
